    About Our Medical Directors

    Dr. Mark Hammond, certified in internal medicine, has been the Medical Director at Kindred Transitional Care and Rehabilitation - Primacy since 2001. Dr. Hammond has a strong history as a long-term care physician, cardiology physician and wound care physician.

    Dr. Hammond received his medical degree from the University of Tennessee Center for Health Services and conducted his residency and internship at the University of Tennessee Baptist Hospital. He is a member of the Memphis and Shelby County Medical Society, the American Medical Association, the Tennessee Medical Association, and the American Medical Directors Association. Dr. Hammond serves on the Board of Directors for the National Medical Enterprises Physician Advisory Board and the American Board of Hospice and Palliative Medicine, is co-chairman of the Non-Invasive Cardiology, and chairman of the Quality Assurance Committee at Methodist Germantown Hospital.

    Dr. Jonathan Ellen, with a specialty focus on orthopedics and physical medicine and rehabilitation, has been Kindred Transitional Care and Rehabilitation - Primacy's Transitional Care Center Medical Director since 2010. He earned his medical degree from the School of Medicine at the Autonomous University of Guadalajara, Jalisco, Mexico, held a sub-internship through the Fifth Pathway Program and New York University School of Medicine, an internal medicine general internship at the New York Hospital of Queens, and a physical medicine and rehabilitation residency at Schwab Rehabilitation Center in Chicago, Illinois.

    Dr. Ellen has served as the Medical Director for inpatient rehabilitation at St. Francis Hospital since 1995, the Clinical Sub Director of neuro-rehabilitation at Select Specialty Hospital since 1998, the wound care team leader at Select Specialty Hospital since 2003, and the consultant for rehabilitation services for Amedisys Home Health since 2005. He was also an associate at Parker Pain and Rehabilitation from 2007-2010.

    Dr. Ellen is the Physician Advisor to the Memphis Brain Injury Resource Group, Co-Chair of Memphis’s “Ride for Your Mind” bicycle ride, and a member of the Memphis Brain Injury Support Group. He belongs to the American Association of Physical Medicine and Rehabilitation, the Tennessee Society of Physical Medicine and Rehabilitation, the Tennessee Brain Injury Association, and the American Board of Independent Medical Examiners.

    When asked what he liked about Primacy, Dr. Ellen said, “Primacy allowed me the opportunity to evolve rehab science from its ground stages and to help in the development of a new program on a sub acute level.”
